This form is the first step for school administrators, athletic directors, and program leads who would like to learn more about bringing a high-performance basketball pathway to their academic institution.

Your responses help us understand your school’s current program structure, vision, and areas where support or enhancement may be needed.

Ministry Standing (Ontario, Canada only)

Is the school registered in Ontario?
Authorized to Grant OSSD Credits?
Are credits transferrable?
Is the curriculum aligned with Ontario Standards?

Please upload one of the following:

  • Ministry of Education BSID / MIDENT Confirmation Letter

  • School’s Name as Listed on the Ontario Ministry Private Schools Registry

  • Annual Notice of Intent to Operate (NOI)

  • School Credit Granting Authority Confirmation

Optional supporting documents include:

  • School Calendar / Timetable Sheet (Official Format)

  • Staff OCT Certificates (at least 1–2 lead teachers)

  • School Insurance Certificate

  • Facility Lease or Ownership

International Student Onboarding Support

Can the school issue study permit letters?
Does the school have a dedicated international student office or support team?
Does the school have ELL / ESL programs?

Athletic Program Support

Is Basketball currently offered?
