1. LIA Basketball Consent & Information Sharing

• I confirm that I am authorized to submit this registration on behalf of the student-athlete.
• I give consent for my child to apply to LIA Basketball and to be considered for participation in its athletic and academic programming.
• I understand that LIA partners with multiple accredited academic institutions, and I consent to the information provided in this application being shared with partner schools for admissions review, placement evaluation, and program coordination.
• I understand that admission, enrollment, placement, and scholarship eligibility are subject to review and may depend on academic standing, program availability, and, where applicable, immigration or study permit requirements.
• I confirm that all submitted information is true, complete, and accurate to the best of my knowledge.

2. Academic Institution Terms & Conditions

• Partner academic institutions reserve the right to withdraw or dismiss a student, without refund and at the parent/guardian’s expense, if application information is found to be false or misleading.
• Partner academic institutions are not responsible for losses or expenses arising from circumstances beyond their control, including but not limited to labour disruptions, weather events, emergency closures, or operational interruptions.
• Partner academic institutions and their representatives will not be held liable for injury, loss, or damage that may occur during school activities, travel, training, or study. If the student becomes ill or incapacitated, the institution may secure medical treatment and/or transportation at the parent/guardian’s expense.
• I agree to indemnify and hold harmless the academic institution, its staff, officers, agents, volunteers, and representatives from any loss or liability arising from my child’s participation, including situations resulting from the student’s own conduct.

3. Assumption of Risk & Liability Waiver (LIA Basketball Programs)

• I understand that participation in LIA Basketball activities (training, practices, competitions, conditioning, and team travel) involves inherent risks, including the risk of injury, illness, or other health-related consequences (including communicable illnesses such as COVID-19).
• I assume all such risks on behalf of myself and the student-athlete (the “Assumed Risks”).
• I release and hold harmless LIA Basketball, its coaches, staff, directors, volunteers, contractors, and representatives (the “Releasees”) from any injury, loss, damage, or expense arising from participation or travel, whether caused by negligence or otherwise.
• I waive any right to claim against the Releasees relating to these risks.
• I agree to indemnify the Releasees for any claims or expenses arising from the student-athlete’s participation, including those arising from the student-athlete’s own actions.
• I confirm that the student-athlete is physically able and medically fit to participate in athletic activity.
• I give permission for photo and video recording of the student-athlete for program documentation and promotional use.
